The Care Coordination Assistant supports the Case Management team in facilitating safe and efficient patient transitions throughout the continuum of care. This role assists with discharge planning, utilization management, authorization support, post-acute care coordination, and communication between healthcare providers, patients, families, payers, and external facilities.
The Care Coordination Assistant serves as a key operational support partner to RN Case Managers and Social Workers by coordinating routine post-acute services, managing documentation, arranging transportation, securing authorizations, and ensuring continuity of care for patients transitioning to the next level of care.
Principal Accountabilities:
- Support RN Case Managers and Social Workers with discharge planning and care coordination activities.
- Coordinate routine post-acute care services in accordance with physician orders.
- Facilitate patient transfers to post-acute care settings, including skilled nursing facilities, rehabilitation centers, home care agencies, infusion services, hospice, durable medical equipment providers, and long-term acute care facilities.
- Communicate with receiving facilities regarding referrals, bed availability, and transfer coordination.
- Ensure all required documentation and medical records are completed and transmitted accurately and timely.
- Arrange medical transportation and confirm logistics with patients, families, and receiving facilities.
- Assist with obtaining insurance authorizations and managing payer requirements.
- Prepare, submit, and follow up on prior authorization requests and supporting documentation.
- Maintain accurate and timely documentation within case management and electronic discharge planning systems.
- Support utilization management and care coordination workflows as directed.
- Provide timely updates to the care management team regarding referral status, barriers, delays, or escalations.
- Assist with departmental data collection and tracking initiatives.
- Collaborate with interdisciplinary healthcare teams to support quality patient outcomes and efficient transitions of care.
- Demonstrate professionalism, initiative, strong communication skills, and attention to detail in all interactions.
Required:
- High School Diploma or GED equivalent required
- Minimum of two (2) years of experience in one of the following:
- Hospital setting
- Medical group practice
- Managed care organization
- Utilization management
- Hospital admissions
- Medical/surgical nursing unit
- Strong organizational, communication, and multitasking skills
- Ability to work effectively in a fast-paced healthcare environment
- Proficiency with electronic medical records and healthcare documentation systems
Preferred:
- Associate degree preferred
- Prior hospital case management, discharge planning, insurance authorization, or care coordination experience preferred
- Experience working with post-acute care facilities and payer authorization processes preferred
- Knowledge of healthcare insurance coverage, utilization management, and discharge planning workflows preferred
