The Geriatric & Extended Care/Home-Based Primary Care & Hospital in Home (HIH) Physician will provide quality provide high quality patient centered care as assigned. Physicians board certified/board eligible in Internal Medicine and Family Medicine are welcomed to apply. Fellowship training in Geriatrics is highly desired. The Physician is responsible for management and oversight of Veterans requiring outpatient geriatric services within the Home Based Primary Care/Hospital in Home & Geriatrics Primary Care Clinic.
