The physician will be responsible for providing primary care to Primary Care (PC) patients however will not have a regularly assigned patient panel but can assume coverage of a defined panel of patients during periods of absence or physician vacancy as defined by management. Will provide direct patient care full time and work as a member of a patient aligned care team (PACT) comprised of a nurse care manager (RN), a clinical associate (LPN or MA), and an administrative assistant.
