Primary Care Physician: The physician is assigned to Primary & Specialty Medicine Service Line to perform duties as a Primary Care Physician in the outpatient clinics. Primary Care Physician will manage scheduled and unscheduled patients in the outpatient clinic setting. Acts as a leader of the PACT team and collaborates with other team members, facility staff and community partners to ensure seamless coordination of care.
