MercyOne North Iowa Medical Center has an excellent full-time employment opportunity for an experienced Nurse Practitioner or Physician Assistant to join the Population Health Services Organization located in Mason City, Iowa.The Population Health Services Organization Post-Acute Advanced Practice Provider (APP) is responsible for the care and oversight of MercyOne attributed patients in the post-acute setting. The provider delivers in-facility and telehealth services that support the identification and care coordination of attributed patients in selected partnered post-acute care facilities. The provider will also be available to provide consultation within the ED and/or acute care setting regarding care transition planning to ensure the most clinically appropriate, least res
