The Licensed Practical Nurse (LPN) provides direct support to the Patient-Aligned Care Teams (PACT) in the Ambulatory Care Outpatient Clinic or Primary Care Telehealth Outpatient Clinic. They are accountable for greeting the Veterans, using different modalities of gathering Veteran data such as observation, vital signs and recording information the Veteran provides them; communicating this data to the Registered Nurse (RN) and/or provider; and taking action as directed.
