LTC Physician Care, PC provides Primary Care (PCP) services to Skilled Nursing and Long-Term Care facilities across Michigan. We are committed to providing exceptional care for patients in skilled nursing facilities throughout Michigan, ensuring their unique healthcare needs are met with expertise and compassion.

LTC is seeking a dedicated Nurse Practitioner to join our team as Primary-Care Providers.

for facilities located in the Greater Grand Rapids Area: Lamont and Forest Hills, MI.

Compensation and Benefits 

• Productivity Based Compensation Model (No Cap)
• $10k SIGN ON BONUS
• 15 PTO days annually
• $2,500 Annual CME allowance plus 3 PTO days to use for CME attendance
• Company paid subscription to “Up To Date”, both mobile and laptop
• Monthly phone stipend
• All medical licensure expenses reimbursed by LTC
• Paid professional liability-malpractice insurance with tail coverage

Requirements and Qualifications

• Experience in Family Medicine, Internal Medicine, or Geriatrics preferred.
• We are happy to consider new graduates.
• Candidates must hold a valid and unrestricted NP medical license in Michigan.  
• Candidates must hold a valid DEA license in the state of Michigan.

Job Structure

• This is an in-person position
• Providers will commute directly between home/facilities
• Nurse Practitioners work in coordination with an LTC Collaborating Physician
• Nurse Practitioner is responsible for the day-to-day care of their patients
• After-Hours on-call is shared among all LTC Providers
