This position is for a board eligible or board certified Internal Medicine or Family Medicine Physician who will split time between the Saranac Lake, NY and Westport, NY Community-Based Outpatient Clinics for the Albany Stratton VA Medical Center. You will function as an integral member of the Patient-Aligned Care Team and will be expected to work closely with team members, including nursing and administrative staff, in the coordination of patient care. If selected, you will practice in our Community Based Outpatient Clinics located in Westport and Saranac Lake, NY. You will be assigned a panel of patients to manage under the Department of Veterans Affairs Primary Care Management Model guidelines. Recommended panel size at the division level is calculated based on patient complexity, support staff, and room availability. You will be expected to provide clinical oversight and participate in peer review of other primary care providers through periodic chart reviews. Your basic responsibilities will include:
Providing patient care.
Acting as surrogate for absent providers when needed.
Attending staff meetings and medical meetings.
Collaborating with and supervising mid-level providers.
Following Veterans Health Administration guidelines.
Timely documentation of encounters.
Working closely with a Patient-Aligned Care Team (PACT) including nursing and administrative staff
Working within your scope of practice.
