JOB SUMMARYThe Hospice Social Worker is assigned a group of patients to oversee social work care for both patients and families, and provides patients and family members with the education and advice needed to cope with issues which arise from various emotional or physical problems as outlined by the Texas State Board of Social Worker Examiners scope of practice. Coordination includes a variety of services including crisis intervention, bereavement, client advocacy, referrals, education, discharge planning and implementation, counseling and psychosocial assessments.Location: Temple, TX -
