The Remote Patient Monitoring-Home Telehealth (RPM-HT) RN Care Coordinator (RNCC) will be responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The goal of RPM-HT is to improve clinical outcomes and access to care while reducing complications, hospitalizations, and clinic or emergency room visits.
