The primary purpose of this position is to provide direct patient care and care coordination, as it pertains, to patients and their care within the Coeur d' Alene Community Based Outpatient Clinic (CBOC) while assigned as a Patient Aligned Care Team (PACT) Nurse Care Manager (NCM) at the Coeur d' Alene CBOC in Coeur d' Alene, Idaho. The PACT NCM/RN RN reports directly to the Nurse Manager of the Coeur d' Alene CBOC. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
