The RNCM functions as part of the Homeless Patient Aligned Care Team (HPACT), which is a comprehensive mental health and primary care team that delivers mental health and medical care to Veteran patients in a longitudinal rather than episodic fashion that has the following focus: prevention, health promotion, health care coordination, and chronic disease management..
