Atlantic Health System is seeking a full-time RN Case Manager for the Case management Department at Over Look Medical Center to work 8am-4pm including 1 weekend shift per month.
Responsibilities
Essential Functions/Responsibilities:
Candidate will have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served.
Through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.
Candidate will demonstrate and embrace the PRIDE core values (Professionalism, Respect, Involvement, Dignity and Excellence) in every aspect of their job.
Coordinates patients through the acute care hospitalization
Manages all aspects of discharge planning/transition process
Performs utilization management and quality screening.
Qualifications
Minimum Education:
High School diploma or GED required
BSN preferred
Licenses/Certification:
Valid New Jersey RN License Required
Minimum Experience Required:
At least two years of acute care experience preferred.
Case Management, Managed Care and/or Home Care experience preferred.
EPIC (EMR) experience preferred.
