The Post-Acute position with Baptist Health is based at River Garden Hebrew Home for Aged, a five-star not-for-profit, mission-driven facility with a long history of excellence in senior care. This is an exceptional career opportunity for a qualified Geriatric, Internal Medicine, or Family Medicine physician to work with Northeast Florida’s leading skilled provider of outpatient, post-acute short-stay rehabilitation, and long-term care. River Garden’s Post-Acute model serves as a national standard with 2 part-time physicians and 2 Advanced Practice Providers onsite. The ideal candidate will have practice experience or training with Outpatient and post-acute patients.
As an Outpatient and Post-Acute physician with Baptist Health, you will have the support of the largest non-profit health system in Northeast Florida and work with the region’s largest network of primary care physicians, spanning over 50 office locations in five counties and 10 cities in Northeast Florida and Southeast Georgia.
