This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. The incumbent will develop and implement a plan to conduct universal screening at both divisions of the VAPAHCS and the CBOCs that addresses the importance of sensitivity and confidentiality, and gender-specific concerns. Will develop training protocols for all primary care and mental health personnel regarding appropriate methods of screening for MST, the use of the MST clinical reminder, and when/how to triage to mental health services for follow-up care of MST-related issues. With experience in trauma-related care, incumbent will provide MST-related mental health services and will be able to assess for high risk factors, acuity and triage to other services/referrals as needed. Develop and implement a plan for identifying and providing MST training to new employees and primary care and mental health trainees that rotate through VAPAHCS and the CBOCs. Will establish and monitor mechanisms to ensure that veterans with experiences of MST have access to appropriate treatment with timely follow-up and will maintain documentation of referral and treatment services provided to veterans, aggregated by gender. The incumbent will consult with VAPAHCS leadership and staff in the Eligibility and Billing Departments to ensure that veterans receiving MST-related counseling and treatment are not charged for inpatient, outpatient, or pharmaceutical co-payments related to MST issues. Interviews veterans and their family members/significant others to establish facts about the veteran's situation, presenting problems and their causes, and the impact of such problems on veteran's functioning and health as part of a comprehensive psychosocial assessment. Interprets and explains VA's treatment and benefit programs. Will participate on related committees (i.e. IPVAP, Mental Health and Women's Health) Incumbent will develop an assessment of the veteran in collaboration with the interdisciplinary treatment team, the veteran, family members and significant others. The goal of the assessment is to highlight the veteran's strengths, limitations, and internal/external supports and service needs to optimize the veteran's functional status. The incumbent demonstrates an understanding of the range of treatment and skills needed for all adult patients related to their physical limitations, psychological needs and age associated illnesses and conditions. Incumbent develops psychosocial treatment plans in coordination with interdisciplinary team members, including goals for psychosocial clinical treatment. Using known available resources and the initial assessment of the veteran, makes initial and continuing decisions regarding use of VA and non-VA services and referrals. Arrives at a reasoned conclusion as to the preferred course of agency action. Provides independent consultation and makes recommendations to interdisciplinary team on course of treatment. Throughout the course of treatment the Incumbent is the subject matter expert on VA and/or community resources.
