St. Luke's Health System in Boise is seeking a Social Worker to join our Virtual Care team.

This is a new position to our system that will support patient care needs in our critical access hospitals via our Virtual Care Center. St. Luke's Virtual Care Center is one of just a few in the nation, a high-tech hub where a centralized medical team uses the latest live, two-way cameras and remote monitoring equipment to diagnose, treat and support patients around-the-clock at clinics, hospitals and homes, often hundreds of miles and many mountain passes away.

A good candidate will be knowledgeable in suicide assessments, safety planning, team dynamics, communication, documentation, crisis intervention and risk screening.

Shifts will be 7:00AM-7:30PM OR 7 PM-730 AM

Our Virtual Care Center advanced technology facilitates and reinforces the sharing of best practices across the entire organization. It rapidly identifies, validates, and spreads adoption of performance-enhancing innovations across its network of contributors and users. The Virtual Center makes the experience and expertise of the entire network easily accessible to help address problems or issues at any particular site within out St Luke's System.

Responsibilities

• Completes psychosocial assessments and plans of care for identified patient populations.
• Identify and provide interventions and goals as indicated by the plan of care.
• Provides crisis intervention services to patients, families, and staff.
• Provides consultation to physicians, healthcare professionals, or community partners in understanding psychiatric, behavioral, social, emotional, and cultural factors related to plan of care for the patient.
• Consults with staff regarding clinical situations; assists inter-disciplinary team in dealing with complex, high-risk or difficult psychosocial situations.
• Serves as a member of a multi-disciplinary team and is a liaison with community resources.
• Provides community resource information and referrals.
• May provide individual, group, or family therapy.
• May provide supervision, leadership, coaching, and/or mentoring to other clinical staff including co-signature of documentation. May provide counter signature of other's clinical practice.
• If state certified, may provide clinical oversight to staff towards advanced licensure.
• May participate in Third Party billing.
• Other duties and responsibilities as assigned.

Qualifications

• Education: Masters degree in social work.
• Licenses/Certifications: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in states of practice
