The VA Care in the Community Nurse (CITC) will assist in planning, designing, integrating, implementing, modifying and evaluating the effectiveness of program components and services to improve the quality of patient care in the community and assist with seamless transition of the veteran back into the VA system. This position is located at the Hruska Federal Courthouse and is aligned with the NWI Chief of Staff. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Physical Requirements: The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. The VA Medical Center is a major interdisciplinary teaching facility, serving medical, surgical, and mental health inpatients and outpatients. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Must be able to lift at least 45 pounds. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. Organizes and coordinates the clinical components for the authorization for Community Care Services in response to the individual health care needs along the illness and care continuum in multiple settings. Centers services around the patient to maximize efficient and cost effective use of health care resources. Reviews all Community Care consult referrals for appropriate justification and clinical appropriateness based on hierarchy of care model. Demonstrates the ability to prioritize and triage workload to address urgent cases in a timely manner. Conducts clinical reviews and documents all pertinent information necessary to pursue authorization and treatment for veterans. Performs and documents care coordination activities to ensure smooth transition for veterans between VA and Community provider teams. Leads team of medical support assistants (MSA's) using patient centered care model to ensure timely scheduling and care coordination for veterans requiring care in the community. Ensures that appropriate medical documentation is sent to treating community providers. Reviews Requests for Additional Services (RFAS) from community providers, coordinates with VACC medical staff as needed and determines additional approvals or denials as appropriate. Ensures all critical values and care coordination with VA and community providers are reported per procedure. Demonstrates flexibility, desire to work with a team, and excellent oral and written communication skills. Promotes excellence in customer service both internally and externally, as evidenced by incorporating ICARE (Integrity, Compassion, Advocacy, Respect, Excellence) values in their daily work practice. Assists as back up to other CITC duties as coverage needs arise.
