The Registered Nurse is assigned to the Ambulatory Care Nursing Service division of Nursing and Patient Care Service and performs nursing duties for all Roseburg VA Health Care System sites of care. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Preferred Experience: 2 years of RN experience is preferred. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. The Ambulatory Care RN Care Manager provides primary care to Veterans and their eligible family members using the Patient Aligned Care Team (PACT) model. This care is provided in the ambulatory clinics located at the Roseburg main campus, Eugene Health Care Center, North Bend VA Center, and Brookings VA Center. The services that are provided may include primary care, case/care management, and other specialty care as designated for the location also provide cross coverage for other areas as assigned by management. Primary Care is the provision of integrated, accessible healthcare services by clinicians who are accountable for addressing a large majority of personal healthcare needs, developing a sustained partnership with patients, and practicing in the context of family and community. RN Care Manager workload consists of intake assessment, health promotion, disease prevention, Women's Health, management of acute and chronic bio psychosocial conditions, referrals for specialty, rehabilitation, and other levels of care, follow-up and overall care management, and patient and caregiver education. RN Care manager provides Direct Veteran care including scheduled clinic visits, walk-in or urgent visits, group visits, telephone visits, incoming telephone requests, triage/place orders by protocol, and manage pertinent clinical reminders. Also, manages Secure Messages from Veterans and emails from consulting health care team members. Conducts virtual or face2face review of Veterans at high-risk of hospitalization, initiate appropriate consultations per protocol, discuss care with specialty consultants, address needs for preventative or chronic disease care, communicate triage to PACT member, review Non VA records, address alerts and complete Follow-up calls with Veterans or family members. Attends and leads daily huddles or team meetings. Provides new Veteran orientation to VA clinic, mentor or precept nurses and students to PACT setting and take active role in Veteran health education/coaching.
