The Patient Aligned Care Team (PACT) Nurse Care Manager (NCM) is a member of the PACT Teamlet (Primary Care Provider, Nurse Care Manager, LPN, Health Technician, and Clerical Associate) and supports the expanded team (Pharmacist, Social Worker, Home Telehealth RN, Psychologist, and Dietician).
