This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Preferred Experience: Home-based Primary Care experience; Knowledge of JACHO Home Care standards, 3 years experience as a home care nurse RN in a VNA or other setting caring for Adult/Geriatric populations, BLS certified and holds a drivers license. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Physical Requirements: The incumbent must be able to drive for extended periods up to 2 hours at a time, be able to perform work that requires potentially heavy lifting/carrying (45 lbs and over), and long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. The incumbent may be exposed to infected patients and contaminated materials and is required to don/doff PPE as clinically indicated in accordance with the infection control policy. ["VA offers a comprehensive total rewards package: VA Nurse Total Rewards The Care Manager Registered Nurse positions are located in the Geriatric Extended Care (GEC) - Home Based Primary Care (HBPC) department with the Lyons VANJ Facility. The VA New Jersey Home Based Primary Care (HBPC) is a special PACT program designed for Veterans with ongoing illnesses that impact their health and functioning on a daily basis. These Veterans, mostly homebound, have difficulty making, keeping or travelling to outpatient primary care clinics because of the severity of their illness(es). HBPC utilizes an interdisciplinary team approach to manage their biopsychosocial needs under the direction of a Primary Care HBPC Provider. Ongoing visits are made, in person or by video to home technology as clinically appropriate, by the HBPC RN and team members to Veterans in their home setting(s) to assist the patient and/or their caregivers with their needs. The RN care manager facilitates care management of assigned patients and works effectively with all levels of staff. Duties include, but are not limited to: Provides care coordination primarily for Veterans under HBPC, including those in the Rural Health Program (RHP) and Medical Foster Home Program (MFH) who access care from the VA Participates in development of individualized plans of care at interdisciplinary care meetings for assigned panel of patients Travels to Veteran homes in New Jersey catchment areas to provide nursing care services that includes: nursing assessments, follow up nursing care interventions, to collect labs specimens via venipuncture, performing wound and/or other cultures as needed, catheterizing patients for urine collection and other reasons, medication reconciliation and education, reviewing fall risk and education on fall prevention for patients and caregivers, providing education on safe home oxygen use, use of glucometers, education on end of life GOCC and review of Advance Directives and Life sustaining treatment preferences, basic home safety and infection control, fire and safety prevention education and reinforcement, pain management strategies, suicide risk assessment and prevention education, emergency management, care coordination via home telehealth (CCHT), clinical video to home telehealth (CVT) and Virtual Video Care (VVC) via video to home technologies approved by the VA Active team member for case conferences weekly, minimally, to set up treatment plans and nursing interventions according to patient/caregiver problems identified and to evaluate outcome of nursing and other interventions Performs role of patient advocate to promote well-being of the patients Performs telephone screening and/or in or outpatient visits to patients to review admission and/or discharge criteria for HBPC, MFH, CCHT, CVT or VVC programs and assist in guidance for setting up equipment for use Provides coverage for other HBPC nurses as assigned Takes part in training and orientation of newly hired nurses to HBPC and associated programs Provides coverage for the HBPC telehealth nurse as assigned and analyzes CCHT data per protocols established by the telehealth program Collaborates with other VA programs, state, local community agencies, and Veteran Service Organizations to access points of care and utilize methods to enhance health care choices of Veterans Assists/performs outreach activities through informational sessions with and outside the VA Coordinates/collaborates with other VA and NON VA departments, agencies and /or healthcare providers to ensure patient needs are met Establishes and maintains relationships and interactions with VA programs and specialty services as well as local community programs to ensure sustained referrals and to generate new sources of recommendations Assists in clerical and administrative tasks as assigned Pay: Competitive salary, annual performance bonus, regular salary increases\nPaid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)\nRetirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA\nInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)\nLicensure: 1 full and unrestricted license from any US State or territory Work Schedule: 7:30 AM - 4:00 PM during orientation, Monday - Friday.
