Rockwood Clinic is seeking an experienced Nurse Practitioner or PA to participate in a multi-disciplinary team to augment the care delivered by our Primary Care providers. The APP in this position will be tasked with partnering with a small panel of patients and their families to develop care plans and focus on improved outcomes.
