The Home-Based Primary Care -Nurse Practitioner is accountable for primary medical responsibility of the HBPC patients. The HBPC program deals with chronically ill Veterans with complex medical problems that cause them to be home bound; frequent interventions help them maintain their optimal level of functioning. The employee works from the parent facility in Altoona, PA or one of the 5 Community Based Outpatient Clinics located in DuBois, State College, Indiana, Johnstown or Huntingdon, PA.
