This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Learn more Social worker is assigned to the Medical/Surgical area at the NMVAHCS and practices in the inpatient medicine units, providing coverage to inpatient surgery and other medical social work program area as needed. The focus of the social worker role is to provide care continuity between the acute setting and community based services. This is achieved through case finding, and coordination of after-care needs based upon integration of medical and social factors in relation to available resources. The social worker is qualified to identify and address the psychosocial issues impacting discharge goals and to ensure linkages between the patients and appropriate community resources. Social Worker also collaborates with clinical and administrative staff in ongoing assessment of internal/external care barriers which may impact upon patient care, length of stay and facility goals. The incumbent functions at the full performance level which requires completion of a minimum of 1 year of post-MSW degree experience and licensure or certification in a State at the independent practice level . The incumbent practices under the NASW Code of Ethics. The Social Worker provides services on an inpatient and outpatient basis. He/She practices under the NASW Code of Ethics. The Social Worker primarily provides services on an inpatient medical/surgical unit, and may provide services in outpatient areas. Independently conducts psychosocial assessments. Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team with specific attention to culture, gender, ethnicity, sexual orientation, stage of life and age-related issues. Social worker possesses the skills to offer psychosocial interventions and measures outcomes to interventions. Social worker is able to coordinate community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. Coordination includes but is not limited to transfers within the NMVAHCS, Veterans Integrated Service Network(VISN), other VA facilities, skilled nursing facilities, rehabilitation facilities, assisted living facilities, hospice and home in conjunction with the interdisciplinary team. Provide case management services to veterans, their families and significant others, including but not limited to, information and referrals, financial and benefits counseling, resource education, linkage to tangible service, and advocacy. Social worker provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology. Establishes and maintains effective therapeutic relationships with veterans and their families.
