Will provide direct patient care full time and work as a member of a patient aligned care team (PACT) comprised of a nurse care manager (RN), a clinical associate (LPN or MA), and an administrative assistant. Practices s as an independent provider providing primary and urgent care utilizing direct face-to-face visits, telephone visits, and veteran video connect visits (VVC). May be assigned to any of the outpatient clinics within the catchment area of the VA Portland Health Care System. Will be the/a primary clinical surrogate for licensed independent provider(s) at their assigned duty station. The employee can also be assigned as the clinical surrogate for any licensed independent provider(s) in the Primary Care Division Work Schedule: Monday-Friday, varying tours from 7:00 am to 5:30 pm
