The Atlanta VA Health Care System is actively recruiting for many Registered Nurses for Geriatrics Nursing Service. The Registered Nurse within the Nursing-GEC service line processes all consults entered for skilled home care (hospice, PT/OT/ Wound care, IV therapy) and unskilled(HHA/Homemaker) services within the Atlanta VA Healthcare System. The homemaker-home health aide program is designed to assist frail Veterans in maintaining their health and well-being in their homes without the need for early nursing home placement. The RN promotes continuity of care and timely hospital discharges by processing consults and subsequent home health placement. The RN acts as a liaison among the VA health care system, Veterans/families, community home health, and hospice agencies. The RN locates community agencies, complete the community care template in CPRS for authorization, brokering, and re-broker agencies as needed. The RN monitors the expiring authorization request report to prevent agency authorization gaps, and troubleshoot vendor issues as required. Performs other duties as assigned by management.
