The Geriatrics/Long Term Care (G/LTC) Service, Community Living Center (CLC) Physician provides inpatient care to CLC Residents. This includes after hours telephone coverage for Veterans admitted to the CLC Houses. The Martinsburg VA Medical Center serves more than 119,000 Veterans across four states; WV, MD, VA, and PA. Located in Martinsburg, WV, the medical center operates seven Community Based Outpatient Clinics (CBOCs). All seven clinics are VA-staffed - located in Cumberland, Hagerstown & Fort Detrick, MD, Petersburg & Franklin, WV and Stephens City and Harrisonburg, VA. The medical center also supports the Vet Center in Martinsburg and the Culpeper and Winchester Cemeteries in VA and the Veterans Hotline in Shepherdstown, WV. Participates as part of the interdisciplinary team to plan the care of the Resident, including resident-specific goals and outcomes for discharge. Provides clinical input and demonstrates leadership as a member of the interdisciplinary team. Participates in interdisciplinary team meetings. Provides education to the G/LTC team and VAMC staff on current geriatric information and clinical practices. Completes appropriate documentation as administratively required such as Minimum Data Set (MDS), referrals, encounters, etc. Completes orders and follows up on results, including lab, radiology, consults, medications, diets, code status and other needs to treat the Resident. Completes routine rounds and evaluations as needed. Mentors rotating medical students, residents and other trainees during G/LTC rotations. Works directly with staff to identify needs, system issues, and communicate goals for the CLC. Will be a leader in performance improvement. Participates in quantitative and qualitative data review to support performance improvement/quality assurance efforts of the CLC. Provides patient education in an interactive setting. Serves on local and VISN committees to represent CLC concerns and initiatives. Willing to work weekends and be on call for coverage in the CLC. Supports the 24/7 coverage system for CLC Residents.
