The Cincinnati OH VA Medical Center is seeking a well-qualified Social Worker for our Geriatrics Department. The employee will utilize the GRECC Connect Program that provides access to geriatric consultation to local CBOC's. The selected candidate will be involved in all aspects of the program development, program evaluation, outreach, data management and research proposals, in addition to being assigned as the geriatric social worker for the GRECC Connect Clinics and providing social work services to the specialty clinics. Duties of this position are, but not limited to: Perform case finding functions through identifying and screening patients for psychosocial needs. Implement treatment modalities Provide counseling or psychotherapy for individuals, families and groups. Develop treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team. Coordinate community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs Conduct psychosocial assessment to determine the cause of distress, often impacting the health condition and creating barriers to care. Treat complicated psychosocial problems of older Veterans and their families or significant others, by using strengths-based, person-in-environment approach to assessment and planned interventions. Assist with administration of the GRECC Connect Clinics, and Geriatric specialty Clinics including updating and maintaining data spreadsheets and assisting with the identification and scheduling of appropriate patients into the clinic. While functioning as an independent professional, the individual is under the direct supervision of the Social Work Chief or delegate and the Chief of Geriatrics and Palliative Care. The social worker will receive clinical and administrative guidance, supervision and mentoring as it relates to Social Work practice.
