Major Duties/Responsibilities: The Patient Aligned Care Team (PACT) physician is administratively responsible to the Chief, Primary Care Service, or designee. The Physician is the clinical Leader for an Integrated leadership team in Primary Care. Provides guidance and collaborates with the AO, MAA, and the Nurse Manager concerning the administrative operations of the PACT clinics. Promotes quality patient care as a clinical expert in Primary Care, and acts as mentor, preceptor, and consultant to the other PACT team members and trainees. Works collaboratively with other health care professionals to accomplish patient, service, and medical center goals, promoting and embracing the Patient Aligned Care Team (PACT) model of care, utilizing Integrity, Commitment, Advocacy, Respect, Excellence (ICARE) values throughout the clinic. The selectee will hold the following competences: Examinations - Uses assessment skills to examine patients thoroughly using appropriate clinical, laboratory and other pertinent data. Ensures the proper follow-up for the assigned panel of patients. Acts in carrying out patient appointments in the assigned time. Diagnosis - Accurately interprets and evaluates clinical data for the purpose of determining diagnosis and clinical status. Therapeutic Ability - Initiates appropriate and timely treatment. Recognizes changes in behavior, symptoms, signs, and other pertinent facts about the patient and has the ability to modify plan of treatment when indicated. Recognizes own capabilities and limitations. Effectiveness in Emergencies - Acts quickly in recognizing emergency situations, and takes timely necessary action. Patient Management - Maintains confidentiality of patients and relatives through competence and interpersonal skills. Acts as a consultant in involving patients in the treatment efforts. Demonstrate the ability to recognize the need for consultation to other facility specialty areas, as well as, does not obtain consult inappropriately. Record keeping - Promptly and completely reports physical examinations and changes in patient status. Maintains clear, accurate, and current clinical records. Completes and signs all medical notes and completes encounters timely. Processes view alerts and other patient notifications in a timely manner; critical alerts with 48 hours, and routine/moderate alerts within 14 days. Generally, does not allow view alerts to accumulate to greater than 100. Complies with Medical Staff bylaws. Prescribes medications according to approved formulary, and in accordance with OBVAMC credentialing, Federal and State law, and VHA regulations. Provides continuity of patient care by performing patient hand-off when transferring care to another physician. Teaching and mentoring - Serves as a mentor in the role of teacher for fellows, resident, trainees, and other employees.
