This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. CBOCs within the Minneapolis VA Health Care System provide care to Veterans near their home town and serve over 18,000 Veterans throughout Minnesota and Western Wisconsin. Seeking a Family Practice or Internal Medicine trained primary care provider is sought for a VA clinic in Superior, WI part of the Minneapolis VA health care system. Our facilities include an updated medical clinic with a lab, and telemedicine capabilities. Our physicians serve the primary care health needs of the Duluth/Superior and rural areas using a team approach with nursing and onsite clinical pharmacist. Mental Health services within the clinic help to provide comprehensive care to the Veterans served in this VA Community Clinic. Take time with your patients. Visits for established patients are 30 minutes and for new patients 60 minutes Providers will perform outpatient duties and management of scheduled and unscheduled patients. An assigned panel of patients is determined by the VHA PCMM software package which is specific to their practice and setting. Focus on care include the chronic disease initiative areas of CHF, COPD and DM, mental health collaboration in the PC setting, MOVE program focusing on weight loss, diet and activity as well as health promotion/disease management.
