Care for people facing a life limiting illness or injury, with a team approach to medical care, pain management, and emotional support tailored to the patient's needs as a Hospice and Palliative Care physician on the CLC/Palliative Care Team within the Geriatrics & Extended Care Service. This position is eligible for the Education Debt Reduction Program (EDRP) Learn more Recruitment/Relocation Incentive available to highly qualified selectee. The Hospice Palliative Care Physician: 1. Is the Attending Physician for the Palliative Care Consultative Team (PCCT). 2. Assures that Palliative Care Consults, Inpatient and Outpatient, are processed and answered timely. 3. Provides a continuum of palliative care services across various settings to include but not limited to: Acute care, Community Living Center (CLC), Outpatient consultative clinics, ED, Geri Palliative clinic as well as the Infusion room. 4. Participates in the care provided to hospice and palliative patients on the CLC to include On-call duties as needed. 5. Participates and is a member of the CLC Screening committee providing medical report on the patients referred to CLC for ongoing hospice and palliative care. 6. Acts as the HPC consultant for the facility providing feedback as needed through e-consults or directly when contacted. 7. Participates in meeting the education needs of medical residents and other staff as identified by the medical center. 8. Assists in developing procedures to ensure adequate palliative care is delivered. 9. Assists in discharge planning to ensure effective transfer of patients from acute care or ED to CLC or discharge home with community hospice services, as necessary. 10. Serves as, member of the Geriatrics & Extended Care Medical Staff, attending meetings, helping to assure adherence to its bylaws, rules and regulations. 11. Acts as the VA Medical liaison for Hospice services with the community as needed. 12. Monitors and evaluates the quality of medical services as an integral part of the overall quality assessments and improvement program and assuring compliance with OIG, JC and Long-Term Care Institute (LTCI) requirements for Long Term Care as it relates to hospice and palliative care services.
