The Hospice and Palliative Care Program consists of an inpatient hospice unit, an inpatient consult service, a palliative care clinic, and is an active teaching program, which spans a variety of disciplines. The incumbent will be one of two social workers with the Hospice and Palliative Care Service. The incumbent will be assigned to working half time in the Community Living Center's 10 bed hospice unit and will be working half time in the outpatient Palliative Care Clinic. Major duties include but are not limited to the following: Screening/Assessment - The social worker will develop an assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members, and significant others. The goal of the assessment is to highlight the Veteran's strengths, limitations, internal/external supports, and service needs, in the context of the Veteran's serious, advanced, and/or life limiting medical issues. Treatment Planning/Goal Setting - The social worker develops a care plan, including short-term goals, long-term goals, Veteran strengths, and end-of-life planning. The treatment plan includes specific goals, objectives, and time frames. Supportive counseling - The social worker provides Veterans and their caregivers with ongoing supportive counseling. The purpose of such counseling is to provide support with the psychosocial impact of coping with chronic/disabling illness (es) and the onset of a catastrophic illness. Adjustment to disability, illness, loss - The social worker will utilize communication and intervention strategies to engage Veterans, aging Veterans (including those with dementia), to facilitate assessment and intervention, to encourage coping with change, and to identify and address caregiver support needs. Crisis Intervention - The social worker is experienced in making rapid assessments of at-risk clients in the community and in recognizing opportunities to intervene to avert crisis or to identify immediate needs and respond to promote patient safety.
