The Cincinnati, OH VA Medical Center is seeking 3 well-qualified Registered Nurses to work in the Home Based Primary Care (HBPC) Program located at either the Bellevue, KY CBOC or the Hamilton CBOC. These RN positions are located in Primary Care. Work Schedule: Monday through Friday, Day shift hours. However, incumbent must be able to change work schedules now and then to accommodate the needs of the service. Financial Disclosure Report: Not required. The duties and responsibilities for the Staff Nurse in the Home Based Primary Care Program include but are not limited to the following: Provides direct nursing care in the home setting. Provides intensive care management for patients. They assume accountability and responsibility for their nursing practice and decisions affecting patient care. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Functions as a case manager, coordinating the patient care with interdisciplinary team members, interacting, coordinating and advocating for the patient/caregiver with other relevant parties across a continuum of care, beyond a single episode of care, ensuring seamless transitions. Assesses the status of admitted patients via home visits, telehealth visits, and telephone visits and develops and coordinates the patient's plan of care to include ongoing assessment and re-assessment, care planning, care coordination, monitoring, and evaluating clinical outcomes. Provides individualized education and support to the patient and caregiver to optimize clinical outcomes and support quality of life decisions. Functions as a preceptor, assuming accountability and responsibility for the clinical development of assigned orientees. Administers intravenous medications in the home per established policies, protocols, and guidelines. Provides in-home physical assessments, point of care testing, phlebotomy, wound care and catheter care in the home per established policies, protocols, and guidelines. Utilizes telehealth technology in providing care to some patients. Responsible for maintaining competency in the use, pre-cleaning and/or reprocessing of reusable medical equipment (RME). Ensuring that the appropriate Standard Operating Procedure (SOP) and manufacturer's instructions are followed for any step in the use of RME (pre-cleaning, reprocessing, disinfection, sterilization, maintenance) in which the provider is involved. Responsible for: post-acute care case management & in-home physical assessments, inclusive of, but not limited to the following: medication management, point of care testing, phlebotomy, wound care, catheter care in the home per established policies, protocols, and guidelines Clinical oversight for a License Practical Nurse, inclusive of and not limited to the following: in home physical assessments, medication management, point of care testing, phlebotomy, wound care, and catheter care in the home This position may support the Federal Government's response efforts to COVID-19. This RN considers all characteristics of the individual, including sex and sex role identification, age and life stage, state of health, race and culture, socio-economic status, values and previous experience with similar events in order to assist each patient to identify and meet basic health needs and improve his/her health status to the individual's maximum potential.
