The Greater Los Angeles VA Healthcare System is seeking 3 experienced and highly skilled Licensed Vocational Nurses (LVN) for our Wound Care service located at our West Los Angeles and Sepulveda campus in the Prevention Amputation Veteran (PAVE) Program. This LVN, with assistance from the RN/Provider, will provide wound, skin and nail care to Veterans admitted to the acute, Inpatient Medical Surgical Units, Inpatient Mental Health units, the Community Living Centers and Outpatient Clinics As a Licensed Vocational Nurse you will provide prescribed medical treatment and personal care services to ill and injured veterans. Duties include documenting observations and changes in patient's condition; collaborating with health team members to facilitate positive patient care outcomes; providing a full range of nursing care to patients with a variety of physical and/or behavior problems; and accountable for meeting and maintaining the qualification standards and functional statements of assigned grade. Our facility is seeking individuals who possess good organizational skills which enable completion of work, demonstrate ability to assume leadership in a patient centered program, and the ability to establish effective communication with patients, families, visitors and members of the interdisciplinary team. Delivers care in an expanded scope for wound care and foot and nail care in multiple levels of care. 2. Evaluates knowledge deficit of prevention of pressure ulcers and diabetic foot and nail care. 3. Evaluates and observes patients, identifying obvious and subtle deviations. 4. Recognizes emergent situations and initiates appropriate interventions. 5. Uses practical knowledge of human motivation behavior and reactions to situations. 6. Acts as a patient advocate in facilitating patient care and ongoing management of self-care. 7. Exercises self-direction in performing the most complex LVN assignments involving both standard and non-standard specialized activities 8. Functions independently in the ongoing provision of nail care, monitoring of sensation and educational needs of high risk Diabetic Veterans. 9. Contributes to the interdisciplinary team in developing and implementing Veteran treatment plans. 10.Assist in the provision of necessary equipment and supplies for the delivery of wound and foot/nail evaluation and care. 11. Participates in PAVE performance improvement activities. 12. Routinely completes encounters and accurate documentation of all provided services. 13. Reviews patient records for preventive care, tracts preventive care and conducts patient education associated with Diabetes and Pressure Ulcer Prevention. 14. Performs other duties as assigned depending on demands and/or needs within the Wound Care Program. 15. Provides care across the continuum of care, Outpatient, PACT, Acute, Inpatient Acute and Inpatient CLC and Hospice as directed. Preferred Experience: Basic computer literacy proficiency with the use of Microsoft Office programs or comparable word processing, spreadsheet and graphic software and the ability to learn new programs specific to the VA. Minimum 5 years of recent, successful clinical nursing practice as an LVN preferred. At least 1 year of wound care nursing experience preferred. Demonstrated evidence of interdisciplinary collaboration, team work, and excellent interpersonal skills. Demonstrated ability to be self-directed and require minimal supervision. Demonstrated ability to work well with patients/families and with other health care professionals. Documented evidence of providing consistent excellent customer service This is a progressive opportunity from a GS 6 to a GS 7 following successful completion of the following: 18 Hours of didactic - (The Aging Foot: An Interdisciplinary Perspective) Ch. 1-15 Basic Foot Care (DVD) Disc 1-3 (4-5 Hours) 80 Clinical hours of proctoring with Attending Podiatrist 50 Patients for basic foot exam and nail care competency; signed off by Attending Podiatrist Daily logs of completed work and progress Monthly meetings with Podiatry Attending and Clinical Personnel Basic Foot Care Course found on TMS - Pre/Post test (3 Hours) Duty Location: West Los Angeles Medical Center ~11301 Wilshire Blvd Los Angeles CA 900 Sepulveda Campus ~ 16111 Plummer St, Los Angeles CA 91343 Work Schedule: Monday - Friday 7:30 Am to 4:00 Pm. 8 hour shift.
