Bay Pines VA Medical Center is a general medical and surgical facility, classified as Level 1A Complexity. It is a teaching hospital, providing a full range of patient care services, with state-of-the-art technology, as well as education and limited research. Providing comprehensive healthcare through primary care, mental health and long-term care. In addition to the main facility in St Petersburg, there are services offered in seven community-based outpatient clinics and one outpatient clinic. The GS-11 Patient Aligned Care Team (PACT) Social Worker performs duties independently and reports to the PACT Social Work Supervisor. The PACT Social Worker works in close collaboration with the PACT team; collaborating with PACT Social Work Supervisor and Section Chief, Social Work Service when necessary. The Patient Care duties of this position include, but are not limited to: Independently performing case finding functions and outreach, identifying and screening patients for psychosocial needs. The incumbent must possess the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups. Social Workers must practice within the bounds of their license or certification. For example, some states may require Social Workers providing psychotherapy to have a clinical level of licensure. Independently assess the psychosocial functioning and needs of patients and their family members and is able to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team. Incumbent possesses the skills to offer psychosocial interventions and measures outcomes to interventions. Conducting psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socioeconomic, cultural, ethnic, educational and other diversified backgrounds. Must have knowledge of human development and behavior (physical and psychological and the differential influences of the environment, society and culture). Coordinating community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. Independently identifies high-risk patients and provides case management services. Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology. Establishes and maintains effective therapeutic relationships with veterans and their families. The incumbent is able to independently work with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills. Independently provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plan. Must have ability to evaluate his/her own practice through professional peer review, case conferences and other organizational means. Communicates effectively, both orally and in writing, with people from varied backgrounds. Must have general knowledge of veterans' benefits and services relating to special programs, service connected compensation, non-service connected pension. The incumbent facilitates referrals based upon veterans' needs and eligibility. Must meet the needs of customers while supporting VA missions. Consistently communicates and treats customers (veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner, and is able to provide the customer with consistent information according to established policies and procedures. Handles conflict and problems in dealing with the consumer constructively and appropriately. Must have a working knowledge and skills in the use of word processing, data management and other computer systems, especially those programs in use by VHA.
