Anticoagulation Nurse

Fairview Health Services is looking for an Anticoagulationnurse to work out of our Kasota location in St. Paul, Mn. This position is approved for 80 hours every two weeks and is benefit eligible.

Provide ongoing monitoring of anticoagulation patients. Provide necessary education and support to patients and caregivers while performing ongoing case management. Monitor progress toward established goals to achieve optimal outcomes. Maintain complete, timely and accurate clinical documentation for each patient.

Provide initial and ongoing assessments of patient, including evaluation of drug therapies, medical conditions, diet and compliance.

• Involve the patient and/or caregiver in developing and implementing the plan of care with specific goals under the direction of the physician.
• Regularly re-assess and revise the plan of care as needed to meet the ongoing needs of the patient
• Utilize the collaborative practice agreement to guide decisions to provide optimal patient care
• Collect patient history and document appropriately

Provide monitoring of appropriate levels for anticoagulation patients.

• Collect and document INR and other lab data from various lab sources
• Integrate all current and prior information to make anticoagulation dosage adjustment decisions and determine follow-up timeframe consistent with patient risk assessment.
• Communicate directly with patient or caregiver.

Communicate effectively with the patient, hospital home care, nursing homes, clinic personnel, pharmacies and physicians regarding care.

• Triage telephone calls from patients and provide appropriate follow-up using established policy/procedure and standards of practice.
• Coordinate communication with other service providers.

Provide education and learning opportunities to pharmacy students on rotation

• Provide guidance, mentoring and supervision to students.
• Utilize students productively while providing and educational experience

Required:

• Graduate of an approved school of professional nursing.

• One year RN experience

• Current RN licensure in the State of Minnesota

• BLS

Preferred:

• BSN preferred, or equivalent in a health related field.

• Previous clinic experience with anticoagulation medications preferred. Telephone clinic triage experience helpful.
