Work Schedule: 7:30am - 4:00pm, Monday - Friday Financial Disclosure Report: Not required The Visual Impairment Service Team (VIST) Coordinator plans, develops, implements, directs and evaluates the Visual Impairment Service (VIST) Team program at the VHA sites within his or her catchment area. The VIST Coordinator has the major responsibility of coordinating all services for visually impaired veterans and service members and their families through the Visual Impairment Services Team. Case management duties may include cases that are both difficult and complex both in terms of the severity of the visual impairment and the presence of complicating physical and emotional factors impacting treatment. The VIST Coordinator functions independently in a number of settings including but not limited to hospitals, clinics, rehabilitation centers, long-term care facilities, educational institutions and home environments. The work environment may be in an office, a clinical setting, classroom, private home, VA facility, nursing home, community sites, and hospitals. The VIST Coordinator case manages patient care and support services for a veteran population which is characterized by a significant number of older patients as well as younger OEF/OIF veterans who may have service-related vision loss. Review of demographic data discloses that the average age of a VA Pacific Island Healthcare System (PIHCS) patient is 69 years with a distribution of 72% male and 22% female. Within these broad categorizations, the VIST Coordinator must tailor patient care and support services to meet the needs of the individual patient. VIST Coordinators are case managers for all eligible Veterans who have visual impairments requiring blind rehabilitation services. These veterans will include legally blind veterans as well as veterans with low vision or other visual disabilities who are not legally blind but have excess disability which can no longer be managed solely by Eye Clinic low vision services and require more extensive vision rehabilitation.
