This is a perfect role for a stronggeneralistLicensed Mental Health Professionalwho also has experience with crisis interventionand who isflexible and eager to use their skills in a new setting. This employment opportunity is at the Transition Clinic located at M Health Fairview St. Josephs Campus in St. Paul. The Transition Clinic is open 7 days per week from 8AM 11PM. The current schedule for day and evening shifts is 8a-4:30p and 3-11:30p. Currently, we are looking for PM coverage during the week and both AM and PM coverage on the weekends. The Transition Clinic is virtual care at this time and both day and evening shifts can be worked remotely. LMHP will also provide crisis assessments for the patients at Woodwinds and St. Johns Emergency Departments and Bethesda. Responsibilities/Job Description This position performs emergency clinical assessment, brief psychotherapy, provider consultation, and crisis support, and connects clients who are in need of care, and/or families, to additional behavioral services. This position works with a multidisciplinary team to provide consultation relating to the patients next level of care. Coordinates care with other medical and mental health professionals, peer support, and entities included in the care of clients. Clinical Services - Provides coordinated care by initiating client centered consultation with internal and external providers involved in care or services. Responsible for obtaining the necessary releases.
- Provides crisis call support and determines appropriate level of care, as needed, for clients.
- Completes a comprehensive behavioral assessment to identify risk, risk mitigation strategies, other patient needs and to recommend a level of care and disposition plan.
- Completes the DEC (Diagnostic Evaluation Center) assessment form for ED patients.
- Determines appropriate disposition, coordinates care, and provides discharge safety and aftercare plans for patients.
- Collaborates with the patient, interdisciplinary team, and family to create a treatment plan based on medical necessity and the patients preference for care.
- Provides primary communication to collaborating partners, e.g. hospital emergency departments, schools, primary care clinics, assisted living, group homes and other treatment providers.
- Adheres to all policies and procedures.
- Maintains knowledge of available community resources and procedures for hospitalizations and non-residential care.
Record Keeping - Obtains all necessary documents, information, and signatures from patients in order to complete assessments and coordinate care.
- Documents risk assessments, de-escalation techniques/suicide prevention, and/or mental status evaluation.
- Promptly and accurately documents patient information, the major crisis or treatment issues and expected outcomes based on medical recordkeeping standards. Documents/charts according to department standards, policy and regulatory and accrediting bodies requirements.
Service to Internal and External Customers - Responsible for responding to queries pertaining to services that can be provided to best meet the needs of the patient.
- Locates and schedules routine and crisis appointments for patients within the established timeframe.
- Establishes and maintains positive and professional working relationships with internal staff, external service providers and other state and county agencies.
- Engages in appropriate professional development through continuing education, membership in professional organization and licensure as eligible.
- As requested, participates in nonclinical activities such as committee work, quality improvement teams, performance improvement and grant initiatives.
Qualifications Required - Masters Degree, from an accredited school, in Social Work, Psychology, or closely related discipline.
- Two or more years experience in crisis counseling/therapy situations;
- Experience serving patients with Mental Health, Substance Use and Co-Occurring disorders
- Basic Life Support Certification
- Valid Minnesota drivers license
- One of the following licenses in good standing in the State of Minnesota:
- Licensed Psychologist (Doctoral or Masters);
- Licensed Independent Clinical Social Worker;
- Licensed Marriage Family Therapist (Doctoral or Masters); or
- Licensed Professional Clinical Counselor (Doctoral or Masters)
- Good working knowledge of electronic health records as well as a comfort level and willingness to utilize computer-based platforms for assessments along with proficiency with computers required. Ability to handle multiple tasks at one time and to quickly switch from one task to another.
- Ability and willingness to provide care at non primary worksite through face-to-face or telemedicine evaluations.
- Depending on area of work, this position requires after-hours rotation and may be scheduled as determined by needs and requirements of the on-call team.
Preferred - Three or more years experience in crisis counseling/therapy
- Two or more years of working within an Interdisciplinary team in mental health care.
This position requires remote work. - Maintains an internet connection consistent with Fairview standards at remote workspace.
- Remain productive and responsive during their scheduled work hours.
- Attend primary Fairview location, as needed.
