·     Home Care JV

·     Long Term Acute Care JV

·     Home Infusion JV

·     Home Respiratory and Equipment JV

·     Achieves operational performance by ensuring compliance with all appropriate internal policies and procedures, and with external regulatory and accrediting agencies

·     Responsible for oversight to assure operational budget performance  

·     Responsible for oversight to assure capital and fiscal accountability

·     Negotiates Joint Venture Contracts annually

·     Responsible for development and Implementation of the Post-Acute JV Program

·     Responsible for JV entity integration with AdventHealth Post-Acute Program

·     Develops Programs with JV leadership to safely transition patients safely from hospital care to post-acute care

·     Responsible for assessing areas if opportunity within each post-acute segment to bring forth recommendations for improvement

·     Effectively collaborates and communicates with regional hospital leaders to assure support is provided to meet performance metrics and goals

·     Responsible to assure collaboration and integration with AdventHealth Preferred Provider Network

·     Hires and retains qualified personnel, when appropriate through effective job design, recruitment, retention, development, performance management programs and processes

GENERAL SUMMARY:

The Program Director of Post-Acute Services is responsible for the development, implementation and evaluation of prioritized Post-acute initiatives at AdventHealth. The scope of Post-Acute services includes Home Health, Long Term Acute Care Hospital (LTACH) Joint Venture (JV) and AdventHealth Home Infusion (AHHI) and AdventHealth Respiratory and Equipment (AHRE).  This Program Director collaborates with the Post-acute leaders to develop and deploy standardized policies and procedures and processes.  The Program Director will develop and deploy education tools and programs that align with overall Goals of the organization.  The measures of success include improvement in clinical, financial and market outcomes that demonstrate value to patients, physicians, payers and the organization. The position is responsible to execute the short-term and long-term goals in a manner that is consistent with the mission. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

EDUCATION REQUIRED:

·    Bachelorâ™s degree in Nursing, Physical Therapy or Health care related field

·    Masterâ™s degree in Health Care or Business related

EXPERIENCE REQUIRED:

·    Five years of Healthcare experience

·    Three years of Leadership experience
