PRIMARY FUNCTION: To assist patients and families to identify social, emotional, and environmental needs and to connect them with available resources/ services through the provision of a full spectrum of discharge planning and social work services. The successful candidate will work 10 hour shifts both Saturday and Sunday three out of four weekends 8am-4:30pm, and a rotating Friday evening. PRINCIPAL DUTIES AND RESPONSIBILITIES:
EDUCATION AND EXPERIENCE REQUIREMENTS: Masters degree in Social Work from an accredited graduate school of Social Work. Licensed MSW required by January 31, 2021. Two years of social work experience in an acute care setting preferred. SPECIAL REQUIREMENTS: CCM required within 18 months of eligibility
